Annotatsiya

Dolzarbligi. Bosh miya o'smalari sababli kraniotomiya o'tkaziladigan bolalarda asosiy anesteziya usulini tanlash metabolik, elektrolit va kislota-ishqor gomeostaziga, shuningdek erta operatsiyadan keyingi davr kechishiga ta'sir ko'rsatishi mumkin.

Maqsad. Propofol va sevofluran asosidagi anesteziya vaqtida ushbu ko'rsatkichlar dinamikasini qiyoslash hamda ularning erta asoratlar bilan bog'liqligini baholash.

Materiallar va usullar. 99 nafar bolada retrospektiv qiyosiy tahlil o'tkazildi: 51 nafar bemorda propofol, 48 nafarida sevofluran qo'llanildi. Anesteziyadan oldin, intubatsiyadan keyin va operatsiya yakunida pH, pCO2, pO2, SBE, ABE, HCO3-, Hb, Ht, K+, Na+, Ca2+, Cl-, glyukoza va laktat ko'rsatkichlari baholandi.

Natijalar. Sevofluran guruhida intubatsiyadan keyin va operatsiya yakunida Na+, intubatsiyadan keyin K+ hamda operatsiya yakunida laktat darajasi yuqoriroq bo'ldi (1,04 +/- 0,34 ga nisbatan 0,86 +/- 0,21 mmol/l; p = 0,0005). Uyquchanlik (16,7% ga nisbatan 2,0%; p = 0,028) va ko'ngil aynishi (22,9% ga nisbatan 3,9%; p = 0,013) ko'proq kuzatildi. Kislota-ishqor muvozanati ko'rsatkichlari bo'yicha farqlar aniqlanmadi.

Xulosa. Sevofluran asosidagi anesteziya elektrolitlar va laktat darajasidagi yaqqolroq o'zgarishlar hamda ayrim erta operatsiyadan keyingi asoratlarning yuqoriroq chastotasi bilan bog'liq.
